During this 90-minute video guest speakers, Mike Mackniak & Sara Valentino of Guardian Ad Litem Services, Inc. of CT give a case presentation and discuss the importance of care coordination. Mike & Sara give pointers on how to be well organized and how to coordinate all of the players involved in a case before it gets too chaotic.
